Read and Agree to the Dental Savings Direct Agreement and enter your Credit Card below

We will not charge your credit card until you visit the office and personally accept the Dental Savings Direct Agreement (but we will perform a ‘pre-authorization’ for your DSD membership plan against a valid credit card. You will not be charged and if you decide not to join after your initial visit, your credit card will not be charged). We will alert you via email after charging your card.

STEP 3:

Visit the participating dentist on your appointment date

* A pre-authorization is a credit card transaction that allows us to make a charge against your credit card in the future, up to the amount displayed during checkout.
